We are seeking a dedicated and experienced Social Worker to join our Clients team within the Adults and Communities service. The successful candidate will provide high-standard social work services to adults and carers. This role will focus on safeguarding vulnerable adults, promoting wellbeing, and ensuring that the social care outcomes align with the Department of Health’s Adult Social Care Outcomes Framework.
Key Responsibilities:
- Carry out individual assessments and support self-assessments using appropriate tools and techniques to identify relevant and cost-effective care options.
- Develop personalised, outcome-led care and support plans that promote long-term independence, choice, and minimise ongoing support costs.
- Conduct risk assessments and Mental Capacity Act assessments, ensuring statutory guidance is followed, and individuals are supported in demonstrating their capacity wherever possible.
- Manage an allocated caseload in compliance with statutory requirements, Council policy, and professional best practices, ensuring the needs and choices of individuals are respected.
- Establish and maintain effective working relationships with local services, specialist teams, and other relevant stakeholders to deliver a holistic and seamless service.
- Promote and safeguard the welfare of vulnerable adults, ensuring compliance with safeguarding policies and procedures.
- Monitor and evaluate the effectiveness of services, contribute to continuous improvement initiatives, and support professional development within the team.
- Understand the budgetary framework of the role, ensuring value for money and cost-effective solutions within defined budget constraints.
