We are seeking an experienced social worker to join the Preparing for Adulthood team within the Learning Disability Service. Ideal candidates will have a strong background in working with individuals with learning disabilities, demonstrating a commitment to improving the lives of vulnerable adults.
Your primary responsibility is to promote the well-being and independence of adults with learning disabilities and their caregivers. You will be actively involved in safeguarding, conducting assessments, and formulating personalised budgets and support plans in alignment with the Care Act (2014). Additionally, offer guidance to service users and carers, and oversee the efficiency and effectiveness of care and support plans.
This role also involves staying updated with changing legislation, contributing to service development, and representing the department in multi-disciplinary settings.
